These centers aren’t just medical facilities; they’re hubs of innovation, designed to provide unparalleled and accessible health care services to our Houston communities.

A New Era of Health Care
HM’s CCCs are all strategically located to serve various communities across Houston, including the recently opened facility in Creekside (serving The Woodlands area). Over the next two years, centers are planned in Friendswood, Missouri City, Spring and Cinco Ranch.

Bringing together a wide range of primary and specialty care services under one roof, our CCCs ensure patients can receive comprehensive medical attention, without the need to visit multiple locations. Services offered may include primary care, cardiology, endocrinology, ENT, OB-GYN, neurology, sports medicine and physical therapy.
Dr. Jordan Dale, chief medical information officer and chief health AI officer, emphasized the importance of these innovations, “Our goal is to leverage technology to enhance the patient experience and improve clinician efficiency. By integrating tools like ambient listening and real-time notifications (see more information below), we’re able to provide a seamless and compassionate care experience that truly puts our patients first.”
Innovative Technologies Enhancing Patient Experience
One of the standout features of our new CCCs is the integration of cutting-edge technologies designed to streamline the patient’s journey and improve overall satisfaction. Some key innovations include:
- Online Pre-Visit Registration
Patients can complete their registration online, before arriving at the center, providing essential information and their medical history in advance. This pre-visit registration process expedites the patient arrival process, reducing front-desk administrative tasks and ensuring clinicians have all the necessary information at their fingertips for each patient’s appointment. - Welcome Kiosks
Upon entering a CCC, patients are greeted by “welcome” kiosks. These self-check-in stations allow patients to register for their appointments quickly and efficiently, reducing wait times and minimizing the need for manual paperwork. The kiosks are user-friendly and equipped with touchscreens, making the check-in process intuitive and accessible for all patients. - Text-to-Wait Updates
To further improve the patient experience, HM CCCs offer text-to-wait updates. Patients receive real-time notifications about their appointment statuses, including estimated wait times and any changes to their schedules. This transparency provides reassurance during appointment delays and helps patients manage their time more effectively. - Integrated Diagnostic and Treatment Services
HM CCCs are equipped with advanced diagnostic and treatment services, including on-site laboratory testing, imaging (X-ray), cardiac rehabilitation, stress testing, advanced heart imaging and EEGs. The availability of these services within the same facility eliminates the need for patients to travel to different locations for tests and treatments, saving time and enhancing the continuity of care.
Promoting Clinician Efficiency

- Real-Time Wait Time Notifications
Clinicians also benefit from real-time wait time notifications, which help them better manage their schedules. By knowing exactly when patients are ready to be seen, providers can optimize their time and reduce gaps between appointments, leading to a more efficient and productive workday. - Ambient Listening
Inside the exam rooms, HM has implemented ambient listening technology. This advanced transcription tool captures conversations between patients and providers, converting them into detailed notes, allowing clinicians to focus entirely on the patient, without the distraction of manual notetaking. Ambient listening not only enhances the quality of care but also helps to ensure that patient interactions are accurately documented.
